O RESPONS\u00c1VEL LEGAL DEVER\u00c1 ENTREGAR OS SEGUINTES DOCUMENTOS NO CENADARTE NA DATA DA MATR\u00cdCULA:<\/p>\n
a) Declara\u00e7\u00e3o de escolaridade (Declara\u00e7\u00e3o da Unidade Escolar informando que o estudante se encontra regularmente matriculado na Rede Municipal de Ensino);<\/p>\n
b) Cart\u00e3o de vacina\u00e7\u00e3o do estudante;<\/p>\n
c) Atestado de sa\u00fade comprovando a libera\u00e7\u00e3o para pr\u00e1tica esportiva;<\/p>\n
d) C\u00f3pia do cart\u00e3o do SUS;<\/p>\n
e) O respons\u00e1vel dever\u00e1 assinar o Requerimento de Matr\u00edcula definido pela Secretaria Municipal de Educa\u00e7\u00e3o;<\/p>\n
f) Telefone atualizado para contato;<\/p>\n
Com rela\u00e7\u00e3o aos atestados m\u00e9dicos: teremos um per\u00edodo para que possam providenciar o documento. No momento da matr\u00edcula, ir\u00e3o retirar, no CENADARTE, a guia com a solicita\u00e7\u00e3o de atendimento m\u00e9dico.<\/p>\n
REPESCAGEM: 12 e 13\/04\/2022 ser\u00e1 o prazo em que os respons\u00e1veis poder\u00e3o efetivar as matr\u00edculas, caso n\u00e3o tenham sido contemplados na primeira e segunda chamadas.<\/p>\n
